Each Proactive Health Assessment includes —

  • Complete Blood Profile & Count

  • Complete urinalysis

  • PSA, TSH and CRP blood testing

  • Body Mass Index (BMI) calculation

  • Coronary risk analysis/serum lipid profile

  • Screening for diabetes

  • Physical Exam - neurological, cardiovascular, respiratory, abdominal, dermatological, and musculoskeletal assessments

  • Cardiac Stress Test

  • Visual acuity

  • Spirometry

  • Abdominal/carotid doppler ultrasounds (areas specific to patient’s age)

  • Breast exam for those over 40 years

  • Lifestyle and Stress Management assessment

  • Outcome & personalized health report

  • Follow up visit with Physician — in-person or via phone

  • Additional add-ons available – please inquire.

Cost is $2,750

In Favour of Preventative Medicine

False Creek Wellness clinic is committed to the highest level of care for our patients, and therefore we are always in favour of preventive care whenever possible. Traditionally, patients seek doctors and health care only after they have an established issue, making it more difficult to treat, and ultimately causing significant discomfort for patients. With preventative medicine, we are able to identify issues prior to any suffering or illness, saving you both physical and mental strain. Discover the difference that a preventative health clinic can provide, and avoid serious ailments before they even start.

Physician’s Note: The Preventative Health Assessment (PHA) is not intended to replace the services of your family physician, but rather complement them. Each PHA takes approximately 4 hours to complete. A Personalized Health Report (PHR) detailing the results and recommendations will be presented to you after the assessment. We encourage you to share these results with your family physician for continuity of care. We do not arrange any additional testing and/or specialist appointments as part of the PHA. If any additional testing or specialist referral is recommended during the PHA, our physicians will discuss the next steps with you in detail so you can make the necessary arrangements through your family physician.

Fees for False Creek Wellness Programs are strictly for the uninsured components of care, which can include preventive healthcare. Physician visits, prescribed lab tests, and any other publicly insured services will be billed to the provincial health insurance plan by False Creek Wellness or by its physicians. Subscription to a False Creek Wellness Program is not a precondition to access insured services. Fees charged by False Creek Wellness for the services above are for non-insured services.
